In order to evaluate the benefits of preoperative chemotherapy in operable breast cancer we performed a prospective clinical study of 150 patients. In all of these patients the treatment of the primary tumour was modified according to the biologic characteristics of the tumour, either by changing the surgical treatment, or the radiation therapy. All of the patients with advanced-stage disease were treated with chemotherapy prior to surgery. The study was carried out by analysing the patient's compliance to the treatment and by evaluating the efficacy of the chemotherapeutic regimen used. The chemotherapy was well tolerated and the rates of response and tumour regression were excellent. Moreover, there was a strong correlation between the histologic response and the recurrence-free survival. The positive impact of preoperative chemotherapy on survival was also seen in patients with positive axillary nodes at diagnosis, thus confirming the importance of preoperative chemotherapy in the treatment of breast cancer.
